Cystic fibrosis (CF) affects roughly 1 in 2,500 to 3,500 live births in populations of European descent — a number small enough to feel abstract. But that birth prevalence is downstream of a carrier frequency that isn't abstract at all: about 1 in 25 to 30 people of Northern European ancestry carry a single CFTR mutation, with zero symptoms and zero way of knowing without testing.
Do the math on any two carriers who happen to pair up, and the odds shift fast — each pregnancy between two carriers carries a 25% chance of an affected child, a 50% chance of a carrier child, and a 25% chance of neither. Most carrier couples have no idea they're playing those odds until a diagnosis forces the conversation retroactively.
What CFTR actually does — and what breaks when it doesn't
The CFTR gene encodes a protein that regulates chloride and sodium transport across cell membranes lining the lungs, pancreas, liver, sweat glands, and reproductive tract. When both copies are non-functional, that transport breaks down, and the body produces abnormally thick, sticky mucus. In the lungs, that means chronic infections and progressive respiratory damage. In the pancreas, it means digestive enzyme blockages and malabsorption. More than 1,900 distinct CFTR variants have been catalogued — with one variant, F508del, accounting for roughly 70% of CF alleles in Northern European populations alone.
The screening panel just got a lot more thorough
For two decades, standard CF carrier screening in the U.S. relied on a fixed panel of 23 well-characterized CFTR variants — the "ACMG-23." In 2023, ACMG updated its recommendation to a much larger panel of 100 specific pathogenic CFTR variants, closing detection gaps that had disproportionately affected people outside the Northern European populations the original panel was built around. That update alone meaningfully improved detection rates for Hispanic, Ashkenazi Jewish, and other populations that the older, narrower panel underserved.

Milder variant combinations can cause isolated issues like male infertility (congenital absence of the vas deferens) without full CF.
Newborn screening has also become near-universal in the U.S., which catches many cases early — but it can't tell prospective parents their odds before conception, and current CFTR modulator therapies work best when treatment starts as early as possible, which is exactly the argument for knowing carrier status ahead of time rather than reacting to a screening result after birth.
